Under the direction of professional nursing staff, provides direct patient care including vital signs, surgical dressing checks, activities of daily living, blood draws, and specimen collection. Observes and communicates patient status, ensuring needs are met. Serves as an active member of the interdisciplinary patient care services team.
Perform testing and manage patient care based on clinical findings.
Record patient care activities, including ADLs (feeding, bathing, dressing, toileting, transferring).
Complete, document, and report vital signs (temperature, blood pressure, pulse, respiration, oxygen saturation, and pain level).
Assist with patient admission, transfer, and discharge.
Promote patient safety, including fall prevention and maintaining a safe environment.
Maintain order, cleanliness, supplies, and linens in patient rooms and work areas.
Provide post-mortem care when required.
Collaborate with the healthcare team to carry out care plans and report condition changes.
Support patient comfort, including monitoring and addressing pain status.
Measure and document dietary intake, urinary/stool output, and patient weight.
Conduct catheter care and apply/remove external urinary catheter devices.
Perform blood glucose monitoring.
Provide wound care, skin care, and report skin integrity issues.
Collect patient specimens for laboratory testing, ensuring accurate identification and documentation.
Assist with orthopedic/rehab device care (braces, splints, compression wraps/stockings, CPM).
Participate in hourly rounding and respond to patient alarms (call lights, chair/bed alarms, tele-sitter, etc.).
Comply with safety standards and report risks, adverse events, or near misses.
Serve as a patient safety companion when assigned.
Use safe lifting/body mechanics and contribute to patient flow and timely care delivery.
Perform other duties as assigned.
Education & Experience
High school diploma or equivalent required.
Current state certification as a Certified Nursing Assistant (CNA) or Geriatric Nursing Assistant (GNA) required.
No prior experience required; healthcare facility experience preferred.
Current Basic Life Support (BLS) certification required.
Knowledge, Skills, and Abilities
Strong verbal and written communication skills.
Ability to work effectively in a team and maintain positive interpersonal relationships.
Strong organizational skills; able to prioritize tasks in a fast-paced environment.
Ability to use computer systems and document accurately.
Proficient in blood glucose testing and documentation.
Ability to perform delegated nursing tasks considering patient age, culture, and individual needs.
